Learn about our services

  • ​Residential Supports help a person to gain or maintain skills to live as independently as possible and fully participate in a community setting of their choosing. It provides up to 24 hours of support, supervision, training, and assistance to maintain the person’s health and safety while remaining in the community.

  • ​Host homes are out-of-home placements for a short or extended period of time within the private home of a trained family. This is an alternative to an institutional or group home setting and may include additional services depending on individual needs.

  • ​Day support services (DSG and DSP) offer non-residential habilitation to prevent isolation and provide daily support, supervision, and skill-building. These services help individuals, whether adults or children, engage with and contribute to their community, often in group settings.

  • ​Day Supports for an Individual (DSI) offers a non-residential habilitation program designed to prevent isolation by providing daily support and skill-building activities. This program supports the individual’s choice to participate in and contribute to their community, ensuring a minimum of one staff member per person. Services can be provided to both adults and children.

  • Provides support, supervision, socialization, personal care, training, and assistance to help people live as independently as possible. This service is often for those who live alone in their own homes, with roommates, or a spouse, and includes help with various activities of daily living necessary for an individual to maintain a self-directed life within the community.

  • ​Chore Services (CHA) provides assistance with heavy household chores to ensure that a person’s home remains clean, sanitary, and safe.

  • ​Homemaker Services (HSQ) provides assistance with light household duties to help maintain a clean and safe living environment for the individual.

  • ​Companion Services (COM) provides one-on-one non-medical care, support, socialization, and supervision for individuals.

  • Item descriptiExtend Living Supports (“ELS”) provide support and supervision to Persons who receive residential support services pursuant to this contract. ELS is provided, as needed, during times when Persons are unable to participate in their normal day activities and supports and functions as an extension of Residential Habilitation Supports (“RHS”), Host Home Supports (“HHS”), or Professional Parent Supports (“PPS”) services.on

  • MTP provides transportation for Persons to and from Day Supports and Supported Employment in a Group services pursuant to this contract.

  • Item description​Personal Assistance Service (PAC) offers personal and supportive services tailored to the specific needs of the individual.

  • Personal Budget Assistance (“PBA”) provides support to Persons with personal financial matters.
